Pain is a complex experience, initiated by sensory information conveyed from a noxious stimulus, greatly modified by affective (i.e. emotional), cultural and cognitive perspectives.
This article provides an overview of the physiological mechanisms of pain and the important pain pathways. We will discuss pain receptors, transmission of pain signals to the spinal cord and pain pathways within the spinal cord. We will also look at how pain can be modulated at different levels along the pathway.
In years past, the prevailing approach to providing pain control was focused on identifying underlying etiologies or pathologic syndromes, e.g., low back pain, trigeminal neuralgia, and cancer pain, that produce the pain.
