How to Change Your Name (7 steps) 1. Determine Eligibility. If you are above the age of 18 and have lived in Illinois for at least six months, you can request a name change by filing forms with the circuit court in your county.

There are statewide standardized forms available from the Illinois Supreme Court's website where you can access all of the required forms and find instructions on how to change your name, for both adults and minors.
Illinois - Request for Name Change (Adult) This form is approved by the Illinois Supreme Court and is required to be accepted in all Illinois courts. In 1, enter your complete current name. In 2, enter the new full name you would like. In 3, enter your complete current address. In 4, enter the date you started living in Illinois.
