PRINCIPAL PURPOSE(S): To officially report the facts and DISCLOSURE: Voluntary; however, refusal to explain the circumstances supporting the assessment of financial charges for circumstances under which the property was lost, damaged, or the loss, damage, or destruction of DoD-controlled property.
