To complete your online application for certification, you must also submit the following documents. Passing Praxis exam scores (sent directly to ASHA from ETS). Official doctoral transcript* which verifies the date and degree awarded (may arrive directly from institution to certification@asha.org).
Supervision must be provided by a New York State licensed Speech-Language Pathologist or Audiologist except experience gained outside New York State or in an exempt setting may be provided by a person with the ASHA Certificate of Clinical Competence.
To successfully complete the supervised experience in Speech-Language Pathology the applicant must meet all the following requirements: Complete at least 36 weeks of supervised experience within any four-year period following completion of the educational program. (A week of acceptable experience is defined as not less than 35 clock hours.)
This form must be submitted directly by the licensed speech-language pathologist or audiologist who will endorse your license application. The Office of the Professions will not accept this form if submitted by the applicant.

Initial Licensure Audiology. Master's degree or higher or equivalent as determined by the New York State Education Department; A Doctoral applicant must compete 1,820 hours of supervised clinical experience under supervision. A Master's applicant must complete 400 hours of supervised practicum and 36 weeks of supervised experience.
