  1. To complete your online application for certification, you must also submit the following documents. Passing Praxis exam scores (sent directly to ASHA from ETS). Official doctoral transcript* which verifies the date and degree awarded (may arrive directly from institution to certification@asha.org).

  6. Initial Licensure Audiology. Master's degree or higher or equivalent as determined by the New York State Education Department; A Doctoral applicant must compete 1,820 hours of supervised clinical experience under supervision.
