Claims Form. This claims website has been set up to help our customers who prefer to submit their claim details online. In order for us to review your claim, please provide the following details:
1. Download and complete relevant claim form(s) 2. Prepare all supporting document(s) 3. Submit completed claim form(s) with all supporting document(s) within 90 days of the consultation/ treatment date. 4. Once all required documents are received, your claim will be processed within 7 working days. 5.
