Pain at this location may be related to stress reaction or fracture, degenerative change, osteonecrosis, tendinosis, or capsular inflammation ( Figs. 3A and 3B ).
The clinical presentation is pain located plantarly over the involved sesamoid with a painful localized callosity in this region. The pain is typically with weight bearing and can be temporarily relieved with orthotic management and paring of the hyperkeratosis.
