The Arizona Department of Revenue revised Form A-4 for 2023 to account for the new flat state income tax of 2.5%. Previous versions of the form are not valid for withholding calculations as of January 1, 2023.

30 Δεκ 2022 · Employees have two options to complete the new Arizona Form A-4 that is required by law for wages paid after January 1, 2023. State employees on the HRIS payroll system can submit an Arizona Form A-4 on paper or electronically via the Y.E.S. website.
Arizona employers are required to make Arizona Form A-4 available to employees at all times and to inform them of Arizona’s withholding election options. Refer to Form A-4 Employer's Instructions for additional details and requirements.
