Right now my itinerary has me meeting a rheumatologist at 7:30, but nothing else yet. For the flight I plan on a fully refundable return ticket so I can move it without fees.

Ideally, this appointment would occur as soon as travel plans are established and at least two weeks — and preferably 4-8 weeks — before departure.
Mayo Clinic appointments are intentionally unrushed. Still, you'll want to use your time wisely to take advantage of your doctor's expertise and experience. Consider flexible travel plans. One of Mayo Clinic's immense strengths is thorough, accurate diagnosis.
