Send a form to each nursing authority, including Canadian authorities, where you obtained a licence/registration (even if the licence is no longer active). Ask them to complete the form and send it by mail or courier directly to NNAS.
The personal information will be used in the administration of your leave and attendance requests. Refusal to complete this form may result in your supervisor not being able to approve the requested leave, and failure to report to duty or absence without authorized leave may result in disciplinary action.
