19 Μαΐ 2024 · To use the heart axis calculator, you have to calculate the net amplitude of the QRS (see QRS morphologies). Subtract the Q wave and the S wave amplitude from the R wave amplitude. If the result is negative, the minus sign must be used (for example, -2). QRS = 12 mm. QRS = 1 mm (R wave: 7 mm - S Wave: 6 mm).

As evident from the figure above, the normal heart axis is between –30° and 90°. If the axis is more positive than 90° it is referred to as right axis deviation. If the axis is more negative than –30° it is referred to as left axis deviation. The axis is calculated (to the nearest degree) by the ECG machine.
To find the Axis in two steps, one must focus on the left side of the ECG. Select the lead with the most isoelectric trace; i.e. the lead in which Q and R waves are of the same height. In the ECG above, that happens to be lead III. The axis will be perpendicular to that lead orientation.
