As part of your application, you will be required to submit a formal training plan* – outlined on Form DS-7002 – which is required by the U.S. Department of State for all interns and trainees participating in the J-1 visa program.
You should apply for new or renewed passport right away. You’ll need to enter passport data to complete the application task, so enter “000000000” as the passport number, and made-up dates for the issue and expiration.
Please make an appointment to see your doctor as soon as possible. Print the health form and ask your doctor to complete it. Return the forms to CIEE by uploading them in your application.
APPLICATION FOR AN INTERNSHIP OR TRAINEESHIP IN THE USA. Congratulations on securing an Internship or Traineeship in the USA. In order to apply for the programme you will need to complete the online application - there are 2 separate parts to this and both are equally important!
