Reemployment Assistance benefits, also known as unemployment compensation, provides temporary assistance to qualified people who are out of work through no fault of their own. The fastest way to receive a copy of your 1099-G Form is by choosing "electronic" as your preferred contact method. You could view the last five years of your 1099-G Form by logging into your Reemployment Assistance account and going to "My 1099-G" in the main menu.
Complete a paper application by downloading and printing from here. The completed paper application can be mailed to ACCESS Central Mail Center, P.O. Box 1770, Ocala, FL, 34478-1770, or it can be faxed or hand-delivered to a customer service center; Applying for public assistance benefits is free.

Claims for Benefits. An unemployed worker may receive information for filing a claim for benefits at any local CareerSource Florida Center operating under your area’s Regional Workforce Board, or by visiting the website for the Department of Commerce at. floridajobs.org. All claims must be filed online.
