I made this template for my own use, but figured I'd circulate it in case someone out there needs something like this. Each sheet can print of 8 playing-card-sized item cards. The layout for this card started as simulation of the fantastic Arcana Note's style of item cards.

I'm looking for fillable item card templates, preferebly optimized for magic items and in PDF if possible. Here is the MS Excel template that Matt uses in critical role. I like Magic Set Editor with Tintagel's D&D 5E Templates . I haven't tried it but you can probably print to pdf file.
While wearing this circlet, you can use an action to cast the Scorching Ray spell with it. When you make the spell's attacks, you do so with an Attack bonus of +5.The circlet can't be used this way again until the next dawn.
