District of Columbia licensees can submit primary name changes electronically through NIPR. Please access the following link: https://pdb.nipr.com/my-nipr/frontend/identify-licensee. How do licensees cancel or surrender a license or line of authority? By submitting a written request via fax to 202-354-1084 and it must be signed by the agent.
