I put together an easy-to-use template Google Docs for Monster Stat Blocks in 5E. Link included. The template looks like this. As promised, the doc link is here. It isn't perfect but it captures the feel of a MM stat block without requiring a program or image embedding.

27 Οκτ 2015 · I've created a form-fillable monster sheet, as the title indicates, that I use for personal use. It uses the layout of the Monster Manual from 5e, with some art changes due to availability. This post is about the desire for such a thing, and even the legality of sharing it.
Epic Miniatures (Ankheg / Forest Monster / Underground Insect / Burying Creature)
It is constructed as a table in google docs, and uses lookalike fonts. The fonts used are broken down in the doc itself, but in general I used Andada and Open Sans, which are available for free (but not default) in docs.
I use it to generate blocks by feeding monster/npc/character data from a db and exporting them to web pages.
