Are you reporting that you were a victim of ID theft? Fill out State Form 57068 and mail or fax it to the department. Upon receipt of this form, DWD will lock the account from further use and initiate an investigation and issue a determination.
Unemployment Insurance (UI) fraud occurs when someone knowingly provides false information, or withholds facts, to obtain UI benefits for which they are not eligible. UI fraud is a serious offense that carries some serious penalties.
Unemployment Insurance Fraud DWD aggressively pursues any act of fraud committed against the Unemployment Insurance Program. DWD cooperates with other state agencies to check employment and unreported earnings. You commit fraud when you: • Knowingly fail to report any earnings during your waiting period, benefit period, orextended weeks;
22 Μαρ 2021 · Employers and claimants can both commit fraud under state unemployment insurance laws. Employer fraud can include certain actions to avoid tax liability or establishing a fictitious employer account to enable fraudulent claims against that account.
Claimants must submit an Overpayment Waiver Request application for consideration. The Department will evaluate the request and issue a determination to the claimant. The application can be found at www.unemployment.IN.gov under “Forms and Downloads.” Certain exclusions apply.
Employers also can report fraud through DWD’s fraud webpage at https://www.in.gov/dwd/2464.htm. Regarding 1099 tax forms issued by DWD for unemployment benefits, DWD is disassociating UI payments from any claimant where there is a confirmed case of ID theft.
