A sinus rhythm is any cardiac rhythm in which depolarisation of the cardiac muscle begins at the sinus node. [1] It is necessary, but not sufficient, for normal electrical activity within the heart. [2] On the electrocardiogram (ECG), a sinus rhythm is characterised by the presence of P waves that are normal in morphology. [2]
Definition (criteria) for sinus rhythm. Regular rhythm with ventricular rate between 50 and 100 beats/min. P-wave with constant morphology preceding every QRS complex. The P-wave is positive in lead II. Figure 1 (below) shows normal sinus rhythm at paper speed 25 mm/s.

Sinus rhythm (which is the normal rhythm) has the following characteristics: (1) heart rate 50–100 beats per minute; (2) P-wave precedes every QRS complex; (3) the P-wave is positive in lead II and (4) the PR interval is constant.
On an ECG, the normal sinus rhythm is characterized by a P wave corresponding to atrial depolarization wave, and it is followed by an isoelectric line where the depolarization wave goes through the atrioventricular (AV) node. After, there is the QRS complex that represents ventricular depolarization.
