All visitors, nationals and residents coming into Costa Rica must complete an electronic epidemiological Health Pass form online before flying. This form can be found in English here https://salud.go.cr. On completion of the form you will receive a QR code.

The pase de salud, or electronic health pass for Costa Rica travel, is simply a form you fill out with your contact, passport and flight information plus the policy number for the required travel insurance for Costa Rica.
Just wondering if anyone knows what specific general health questions are asked, other than ones relating to covid symptoms, for the required health pass. Do they delve into personal habits, past conditions, general medical history?

17 Σεπ 2021 · Costa Rica has introduced a new health declaration form. This was done to help prevent the spread of COVID-19. All travelers traveling to Costa Rica must now complete a Health Pass before entering the country .
