No testing required; No vaccination required; No “pase de salud” electronic health pass required; No travel insurance required; No QR codes required
All visitors, nationals and residents coming into Costa Rica must complete an electronic epidemiological Health Pass form online before flying. This form can be found in English here https://salud.go.cr. On completion of the form you will receive a QR code.
It will be free to check in on Monday (24hours before departure), however I would like to know: i) Is the health pass immediately activated/issued? ii) Does anyone know what further information is required on successive pages of the form? iii) Does the health pass need printing out ( we probably don't have access to a printer then)?

4 Νοε 2020 · Requirements to enter Costa Rica: Fill out the electronic epidemiological HEALTH PASS form, available at https://salud.go.cr. Passengers will be required to present the QR code that is generated from completing the Health Pass up on arrival into Costa Rica.
4 Οκτ 2021 · mulhall-sk, I am not sure if you have downloaded your digital proof of vaccination - https://covid19.ontariohealth.ca before trying to complete the Pass linked in my earlier reply. Hope you can figure it all out and will get your Code.
