All visitors, nationals and residents coming into Costa Rica must complete an electronic epidemiological Health Pass form online before flying. This form can be found in English here https://salud.go.cr. On completion of the form you will receive a QR code.

If you're going to travel to Costa Rica, you'll need to fill out an electronic health pass form and get travel insurance for Costa Rica that meets the travel requirements set forth in the health pass form.
4 Νοε 2020 · Requirements to enter Costa Rica: Fill out the electronic epidemiological HEALTH PASS form, available at https://salud.go.cr. Passengers will be required to present the QR code that is generated from completing the Health Pass up on arrival into Costa Rica.
