Health Pass (Pase de Salud) All visitors, nationals and residents coming into Costa Rica must complete an electronic epidemiological Health Pass form online before flying. This form can be found in English here https://salud.go.cr. On completion of the form you will receive a QR code.

Answer 1 of 5: Just wondering if anyone knows what specific general health questions are asked, other than ones relating to covid symptoms, for the required health pass. Do they delve into personal habits, past conditions, general medical history?
Costa Rica Health Pass. 2 years ago. We completed all the information and uploaded proof of vaccination BUT the site won't give me a QR code without putting in that we have traveller insurance which is not needed as we are fully vaccinated.
