Author names and affiliations should be present at the beginning of the manuscript underneath the title. Each affiliation should have a label, and the labels should be present by the authors’ names, as shown in the example below.

Adhere to the proper format: The affiliation letter must be printed on institutional letterhead and must be signed by the author. Title Page. The title page should include the article title, author name (s) and permanent affiliation (s), and the name, current address, email address, and telephone number of the person to whom page proofs and reprints should be sent. Abstract. The abstract should appear on a separate page, immediately following the title page.
