Family Health West Hospital Transitional Care in Fruita, CO. At Family Health West, we provide Skilled Nursing (Swing) Beds in our Critical Access Hospital setting. Patients are admitted under Skilled Nursing criteria which allow us to provide on-site Physical, Speech and Occupational Therapies.
