Many new patients prefer to complete patient registration using Mayo Clinic Patient Online Services. Log in to your online services account, or create a new account. Under the Billing and Insurance menu, select "Insurance Information" to complete your patient registration online.
Creating Your Patient Portal Account. To set up a patient portal account, you will need to open the activation email you receive from Mayo Clinic. Your email will contain an online form and an activation code that is unique to you.
Follow these steps to sign up for a Patient Online Services account. Enter your personal information. Verify your contact information. Choose a username and password. If you have any questions, please contact us at 877-858-0398.

22 Μαρ 2020 · Follow these steps to create your patient portal account: Go to the portal, and select "Create your account." Complete the online form to begin your account creation. You will need your Mayo Clinic number, which can be found on letters from your provider, appointment reminder letters or billing statements.
When you’re at the clinic or our hospitals, feel free to ask a registration staff member to help you find the number. Create Your Account 1. Open your Web browser. 2. Type in mayoclinichealthsystem.org . You’ll be taken to the Mayo Clinic Health System’s website. 3. Click on Patient Online Services in the blue bar on the top of the page.
