If you decide to file a discrimination complaint, you must do so within 15 days from the day you received notice from your EEO Counselor about how to file a complaint. This notice is sent to you after your final interview with the EEO Counselor.
If you are a federal employee, former federal employee, or job applicant for a federal job, and you believe that a federal agency has discriminated against you with regard to employment, you have a right to file a complaint.
If you are a current employee of the Court Services and Supervision Agency, a former employee, or an applicant for employment and you believe that you have been discriminated against in a personnel practice or in a term, condition, or privilege of your employment, you may file an EEO complaint.
On the remainder of this form, please answer the questions relating to your problems with the defendant(s) and, if necessary, include additional sheets of paper in order to fully explain the facts beyond your complaint.
