If the knee cannot be flexed more than 30 degrees to 40 degrees, we perform a quadriceps release rather than a V-Y turndown or tibial tubercle osteotomy. A V-Y turndown devascularizes the extensor mechanism, and adjusting the final length of the quadriceps mechanism is difficult.
At 60-90 degrees of knee flexion, the patella has maximum contact with the intercondylar groove (1/3 of the posterior surface area of the patella). At 135 degrees of knee flexion, the patella contacts the femur with its superior pole and rests below the intercondylar groove.
