  1. Provider Enrollment is comprised of three business units, the Managed Care, Facility, and Home and Community-based Services Unit, the Practitioner and Supplier Unit, and the Provider Eligibility and Compliance Unit.

  2. Provider Enrollment is responsible for enrolling qualified providers to receive Medicaid reimbursement for services rendered to Medicaid Fee-For-Service recipients. Florida Medicaid's Web Portal solution provides communication and self-service tools to the provider community.

  3. Once the Enrollment status is Active, the provider receives a Welcome Letter, and Florida Medicaid ID. Full and limited enrolled providers will also receive a PIN Letter, that will be used to create a secure web portal account.   4. To access the Interactive Enrollment Checklist, visit mymedicaid-florida.com. From the homepage, hover over the Provider Services tab, and click Enrollment. Once at the Provider Enrollment page, look under the New Medicaid Providers section, and click Interactive Enrollment Checklist.

  5. PIN Number — this is the 10 digit Personal Identification Number (PIN) that is assigned to each individual within a case. Status — this displays the current state of the customer’s case: • Processing — this status is displayed when the case is currently being reviewed for eligibility.

  6. Some people have a Florida PIN ID, a Current Medicaid ID and an ORIGINAL Medicaid ID. Providers should always bill using the Medicaid ID listed on the person’s approved service authorization. Tier Assignment: Tiers 1, 2, and 3 denote the HCBS Waiver. Tier 4 denotes the FSL waiver.

  7. To track the status of your enrollment, you may go to Florida Medicaid Web Portal, enter your Application Tracking Number (ATN) and the Business or Last Name exactly as submitted on the application, including punctuation, in the search tool.
