In the last few seasons, a chest appeared under that set of stairs 100% of the time. You could count on it being there. A few seasons back (and brought back again), they have removed the 100% spawn rate. So, that same spot under the stairs will usually have a chest, but not always.
Don’t shoot the messenger but they got rid of 100% chest spawn rates because a certain sub-community of the Fortnite population complained endlessly about people splitting drops and effectively cheating.

It should not be 100% spawn. Reason being it incentivizes people to go to places like Tilted. If someone can go to a place like westworld that would have 17 guaranteed chests or Motel which has like 8 guaranteed chests, they can get the same loot there that they would get hot dropping.
