In the last few seasons, a chest appeared under that set of stairs 100% of the time. You could count on it being there. A few seasons back (and brought back again), they have removed the 100% spawn rate. So, that same spot under the stairs will usually have a chest, but not always.
Making chests 100% spawns would fuck up the game at its core, not just the meta for a while. TLDR: The game and all of its map was designed with chests not being 100% chance in mind, and reworking all of that changes a lot more than just the meta for a couple weeks.
