A pplicants can review the status of their application by going to the Georgia Web Portal homepage at www.mmis.georgia.gov and clicking on the Provider Enrollment/Enrollment Application Status link. Scroll down to the bottom of the page and enter the ATN and Business or Last Name.
Welcome to the online Provider Enrollment application. The enrollment application is a one source application for both fee-for-service Medicaid and CMO (Care Management Organization) enrollment. You must complete each step in the Enrollment application.
6 Φεβ 2014 · To improve efficiency and timeliness of the revalidation process, DCH will require all providers to revalidate online only using the Georgia Medicaid Management Information System (GAMMIS) at www.mmis.georgia.gov. Providers may not revalidate using a paper application.
How to Complete Medicaid Application: As an existing provider, you may need to submit the DCH Georgia Department of Community Health (DCH), Division of Medical Assistance (DMA) Additional Location Application (3-pages). Providers who meet staff and business requirements are invited to complete the application process. Those providers who successfully complete the application process are then eligible to serve people with developmental disabilities in Georgia who receive services through Medicaid waivers.
