An antimicrobial app and/or online calculator is available and should be used to calculate the initial gentamicin dose. STEP 1: . Calculate, prescribe and administer the first dose. To reduce the risk of mortality, commence gentamicin administration within 1 hour of recognising sepsis.
Calculate the initial dose by multiplying the patient’s actual body weight (or corrected dosing weight if the patient is obese) by 2.5 (i.e. 2.5 mg/kg ) to a maximum of 180 mg. Alternatively a SINGLE dose of 7 mg/kg may be considered (do not repeat this dose).

IV Injection (not suitable for once daily dosing) IV bolus over 3 - 5 minutes undiluted. IV Infusion. Add the total dose of gentamicin to 100mL of infusion fluid and administer over 20 minutes. Preferably administer via a central venous access device to avoid potential venous irritation.
