An antimicrobial app and/or online calculator is available and should be used to calculate the initial gentamicin dose. STEP 1: . Calculate, prescribe and administer the first dose. To reduce the risk of mortality, commence gentamicin administration within 1 hour of recognising sepsis.
Dose MUST be individualised according to renal function and weight - the gentamicin dosing calculator on the GAPP antimicrobial app should be used to calculate the dose. Dose should not normally exceed 400mg for once daily dosing regimen.

Use the Interval Calculator to determine the timing of subsequent doses after a gentamicin level has been measured. A gentamicin level should always be taken after the first dose. Levels must be taken 6 – 14 hours post dose. In stable renal function, if course length is > 3 days, levels can be taken twice weekly. Dose Calculator.
Calculate the initial dose by multiplying the patient’s actual body weight (or corrected dosing weight if the patient is obese) by 2.5 (i.e. 2.5 mg/kg ) to a maximum of 180 mg. Alternatively a SINGLE dose of 7 mg/kg may be considered (do not repeat this dose).
If creatinine is known ‐ use the online gentamicin dose calculator. If creatinine is not known ‐ give 5mg/kg gentamicin (maximum 400mg) or, if CKD 4 or 5, give 2∙5mg/kg (maximum 180mg) on advice of senior medical staff.
Box 2: Initial GENTAMICIN dose. If creatinine clearance is >20mL/min and the patient is not obese, calculate the dose by multiplying actual body weight (kg) by 7 (i.e. 7mg/kg) to a maximum dose of 600mg. Round this to the nearest 40mg for ease of administration (see Table 1).
