To request an appointment with one of our primary or specialty care providers, we offer four convenient options: Patient Portal: Login or create an account now to request an appointment. It’s easy! Use our Nurse Hotline – just call 855-HAYSMED (855-429-7633). Our nurses are available Mon-Fri: 4:30 pm – 8:00 am, 24 hours on weekends and holidays.
