3 Ιουλ 2023 · CrCl 15 to 29 mL/min: Initial dose: 300 mg orally in the morning on day 1 and day 3; then 300 mg once a day in the morning; may increase to 300 mg orally 2 times a day if needed. CrCl less than 15 mL/min: 300 mg orally every other day; increase to 300 mg orally once daily if needed.
In adults with postherpetic neuralgia, gabapentin capsules may be initiated on Day 1 as a single 300 mg dose, on Day 2 as 600 mg/day (300 mg two times a day), and on Day 3 as 900 mg/day (300 mg three times a day).
