Introduction. In November 2008, Fannie Mae and Freddie Mac released the Market Conditions Addendum Form—Freddie Mac Form 71/Fannie Mae Form 1004MC—which was revised in March 2009. This form will be required on appraisals with effective dates of April 1, 2009 and beyond.
