Use this form: • If you have premium Part A or Part B, but wish to no longer be enrolled. • If you have Part B, but recently re-joined the workforce with access to employer-sponsored health insurance and wish to voluntarily terminate this coverage.
If you change your mind about cancelling your coverage, you need to contact SSA before the date your coverage ends. Contact your local Social Security office. If you’re dropping Part B and keeping Part A, we’ll send you a new Medicare card showing you have only Part A coverage.
You can voluntarily terminate your Medicare Part B (Medical Insurance). However, you may need to have a personal interview with us to review the risks of dropping coverage and for assistance with your request.

Cancel Part A or B. Fill out Request for Termination of Premium Hospital Insurance of Supplementary Medical Insurance (Form CMS-1763) (PDF) and fax or mail it to your local Social Security office. You can cancel Part A only if you pay a premium for it. You can cancel Part B at any time.
