Proof of Social Security Number (full number required, no photocopies) The name on your Proof of Social Security Number must be similar to your identity document or name change document. For example, the name on your SSN card must be similar to the name on your marriage certificate.
To apply for a REAL ID, you must present several documents, including ONE proof of identity document from the list below. This document must include your date of birth and your full name (first, middle, and last). Check mark the document below that you’ll use as your proof of identity.
Two different documents are required from the list below. • Home utility bills (including cellular phone) • Deed or title to residential real property, property tax. • Medical documents bill or statement, original copy of an approved claim for Homeowners’ Property Tax Exemption (BOE-266) form.
A REAL ID is a new form of identification that has its own documentation requirements. Even if you previously submitted your proof of identity documents to DMV for a driver’s license or identification card, you must do so again in order to meet federal REAL ID application requirements.
If you want to apply for a REAL ID, you must also provide proof of your identity, SSN, and two proofs of residency from the list of acceptable REAL ID documents. You will receive your new ID card via mail within three to four weeks.
You can use this form to apply for, renew, replace, correct, or update a driver’s license or identification card, or to apply for license endorsements.
