There are four ways to apply for Medicaid: Online. You can apply online at https://abe.illinois.gov. Applying online is the best way to keep track of what information you submitted in case you need to make any changes. When you apply online, you will be asked to create a secure account.
You can use ABE to apply on-line for healthcare, SNAP, cash assistance and the Medicare Savings programs with the State of Illinois. You can also apply for healthcare and SNAP over the phone by calling the ABE Customer Call Center (800) 843-6154

State of Illinois has a new web-based application portal for Medicaid, SNAP and case benefits. We call it ABE, the Application for Benefits Eligibility. This overview will familiarize you with some of ABE’s features and explain how to set up your user account and submit an application.
Instructions for Mail-In Application for Medical Benefits (This application is available in Spanish.) Medical benefits are available to eligible persons who need help paying their medical bills. This is NOT an application for cash assistance, food stamps, or the other programs listed on page 6 of these instructions.
Start a new application for Health care coverage, SNAP, Cash Assistance, and/or Medicare Savings Program. For most people, it will take approximately 30 minutes to fill out the application. Keep working on an application that you have already started.
