This approximately occurs at vertebral level L5-S1. The artery descends inferiorly, crossing the pelvic inlet to enter the lesser pelvis.

From the aorta, each common iliac artery travels down about 1 inch (3 centimeters) before it divides. There, it splits into an internal iliac artery and external iliac artery. These smaller channels of the iliac arteries divide into even smaller arteries to reach more of your lower body.
Structure. The internal iliac artery supplies the walls and viscera of the pelvis, the buttock, the reproductive organs, and the medial compartment of the thigh.
