HEALTHWAVE 21 PREGNANCY REPORT FORM Rev. 10/03 This form is to be used to report when a HW21 beneficiary is pregnant. It should also be used when an eligibility worker learns of a change in due date or that a pregnancy terminated early. Email form to EDS at t21.duedate@ksxix.hcg.eds.com.

1 Μαρ 2001 · Data analyzed in this report consist of enrollment records from the Kansas Department of Social and Rehabilitation Services (SRS) for all children in HealthWave and Medicaid who are under 19 years of age and are not disabled.
HealthWave coverage will be processed and managed by Clearinghouse staff. Families accessing other benefits besides such medical coverage, including food stamps, child care, and other medical assistance, will be handled by the appropriate local SRS office. Further administrative details are covered later in this memo.
The KanCare program is the State of Kansas' managed care program. KanCare is provided to all Medicaid and CHIP consumers. Kansas has contracted with three health plans, or managed care organizations ( MCOs ), to coordinate health care for nearly all beneficiaries.
