The HealthWave program was developed to provide comprehensive, quality health coverage for low income children and families across the state of Kansas.
Employees with Kansas source wages hired after Dec. 31, 2007 are required to complete and submit Kansas Form K-4 to their employer on or before the date of employment. The Kansas Form K-4 is optional for employees hired prior to Jan. 1, 2008.
HealthWave coverage will be processed and managed by Clearinghouse staff. Families accessing other benefits besides such medical coverage, including food stamps, child care, and other medical assistance, will be handled by the appropriate local SRS office. Further administrative details are covered later in this memo.

Purpose of the K-4 form: A completed withholding allowance certificate will let your employer know how much Kansas income tax should be withheld from your pay on income you earn from Kansas sources. Because your tax situation may change, you may want to re-figure your withholding each year.
The State Supplemental Payment Program was created by the 2006 Kansas Legislature with passage of Senate Bill 62 and subsequent enactment of K.S.A. 39-972. This legislation actually made several modifications to long term care assistance for Medicaid eligible individuals.
