I'm of the notion that kenjaku either was either in the body before the pregnancy of yuji, or took it over during it. I feel like kenjaku made yuji with the intent of making him sukunas vessel, which is why he was so easily able to slide right into the role of it after he consumed the first one.

After reading chapter 143 and 160, my understanding was that Itadori's biological mother was one of Kenjaku's more recent hosts. She (He?) came into Jin Itadori's life after his childless first wife Kaori's death and then had Yuji.
