The limited specificity of Light’s criteria (ie, misclassification of transudates as exudates) can be overcome with the examination of the serum to pleural fluid protein gradient, the serum to pleural fluid albumin gradient, or the pleural fluid levels of N-terminal pro-brain natriuretic peptide.
Light’s criteria were developed to discriminate between transudative and exudative pleural effu-sions, but should not be used with peritoneal or pericardial fluid for which they have no value. Light’s criteria virtually identify all exudative pleural effusions and reveal many clinically unsus-pected transudative effusions.
