Introduction. This document describes the protocol for patients prescribed lithium to be managed safely in primary care, secondary care and across the interface.
Patients prescribed lithium are monitored in accordance with best practice guidance. Prescribers check that blood tests are carried out and monitored regularly and that it is safe to issue a repeat prescription. The responsibility for ensuring that monitoring takes place lies with the prescriber.
Patients prescribed lithium must have a medication alert activated on their electronic patient record and must be easily identifiable by search on GP clinical records (i.e. lithium added to their repeat prescription) .
The Lothian lithium guidelines include a lithium treatment plan (see Fig. 1); advice for clinicians on managing patients' lithium levels; and pointers for counselling patients on lithium (to be used in conjunction with a patient information leaflet).

Prescribers and pharmacists check that blood tests are monitored regularly and that it is safe to issue a repeat prescription and/or dispense the prescribed lithium. Systems are in place to identify and deal with medicines that might adversely interact with lithium therapy.
patients who are prescribed lithium should be monitored in accordance with NICE guidance at the start of lithium therapy and throughout their treatment. Patients should receive appropriate on-going verbal and written information and a record book to track lithium blood levels and relevant clinical tests. Prescribers and pharmacists should check
