Medicare covers skilled care in a skilled nursing facility (SNF) under certain conditions for a limited time. Skilled care is health care given when you need skilled nursing or rehabilitation staff to manage, observe, and evaluate your care.
I. Preamble. The California Standard Admission Agreement is an admission contract that this Facility is required by state law and regulation to use. It is a legally binding agreement that defines the rights and obligations of each person (or party) signing the contract.
The California Department of Public Health has posted the California Standard Admission Agreement for Skilled Nursing Facilities and Intermediate Care Facilities (CDPH 327) on its website in English, Spanish, Chinese, Korean and Vietnamese. The Standard Admission Agreement consists of the basic Agreement and the following attachments:
