To effect a name change as an adult outside marriage or divorce grounds, interested persons, 18 years or older, must file a Petition for Change of Name (Adult) or Form CC-DR-060 in person at a local Circuit Court. They must attach their birth certificates or other official documents reflecting their present name.

The law requires that you notify the MVA within 30 days after you change your name by court order (divorce) or by marriage.
