Pennsylvania’s Medical Assistance (“Medicaid”) program includes home and community-based services waivers (“HCBS Waivers”) that fund a variety of services enabling eligible individuals with intellectual disabilities (“ID”) and/or autism to live in their own homes and communities.
Medical Assistance (MA) is also known as Medicaid). It is a public health insurance program administered by the state of Pennsylvania’s Department of Human Services (DHS). MA provides a comprehensive benefit package and has very little cost-sharing.
Medicaid Provider Forms. The Office of Medical Assistance Programs (OMAP) produces and distributes over 70 forms and envelopes for use at no charge to Medicaid providers. There may be a limit to how many forms can be ordered at one time.
