Download and fill out these forms: Petition for Change of Name ( form NC-100) Name and Information about the Person Whose Name is to be Changed ( form NC-110 ). You need 1 of these attachments for each person whose name you want to change.
There are two main ways to change the name of a child: q Fill out and file these court forms to ask the court for a name change: – Petition for Change of Name, NC-100. – Attachment to Petition to Change Name, NC-110. – Order to Show Cause for Change of Name, NC-120.
