How to fill out forms to change a child’s name. Download the forms or fill them out online. Download and fill out these forms: Petition for Change of Name ( form NC-100) Name and Information about the Person Whose Name is to be Changed ( form NC-110 ). You need 1 of these attachments for each person whose name you want to change.

CIVIL PETITION FOR NAME CHANGE – Instructions. Between 9:30 a.m. and 11:00 a.m. present your completed forms to the ex-parte calendar held Room 210 (Probate Examiners) of the Wakefield Taylor Courthouse at 725 Court Street, Martinez, California.
This program will help you fill out the forms needed to ask the court to change the name or name and gender of an adult, the name of a child, or the name of a family group, and includes a fee waiver application if you need one.
The process is very simple. You will have to file a Petition, get a court date, put a notice in a newspaper about the court date, notify the other parent that you are asking to change your child’s name, and go to court. The rest of this handout explains all the steps in more detail.
You can fill out the court forms online at: www.courtinfo.ca.gov/selfhelp/other/ namechange.htm. Then, go to a court hearing and ask the judge for a court order with the child’s new name.
