Milrinone (Inocor-Sanofi-Winthrop) represents a second generation phosphodiesterase inhibitor currently approved for intravenous administration in the treatment of decompensated congestive heart failure.
Milrinone inhibits phosphodiesterase 3 in cardiac and smooth muscle tissues, thus increasing cyclic adenosine monophosphate levels and a resultant increase in intracellular ionised calcium. It is a positive inotrope and a systemic and pulmonary arterial vasodilator, with little positive chronotropic effect.
